Please join us for a presentation by Agnes Grudniewicz: Addressing a Growing Health Care Problem: Complex Patients in Primary and Community Care.
Complex patients are individuals with multiple chronic conditions, which often compounded by psychosocial needs. Care for these frequent users of the health care system tends to be costly and requires coordination across health and social organizations, resulting in system and clinician burden. Dr. Grudniewicz will present her research program targeted toward this growing challenge faced by health systems around the world. She will present an overview of a series of case studies of the Health Links, a provincial integrated care initiative, from a complex adaptive systems perspective and discuss their implementation from an organizational and systems-level. The presentation will conclude with the clinician perspective to the management of complex patients, focusing on goal-oriented care as a strategy to improve patient outcomes.
